You will:
Design and implement patient experience and workflow of telemedicine and concierge services
Develop service policies and procedures, and facilitate safe professional practice environment to improve the quality of care delivery
Communicate and coordinate with international hospitals for preparation, submission and consultation of all cases
Follow through and actively manage all telemedicine cases from beginning to completion
Serve as a single point of contact for referring physicians, patients, caregivers to provide resources and assistance with accessing clinical supportive care services offered within Jiahui International Hospital
Navigate and coordinate external and internal services to ensure timely and satisfied service delivery to cancer patients
Introduce care plan and services to patients including pricing and product offering content
Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team
Evaluate new patient cases and develop plans of care
Implement, and evaluate individualized patient care plans
Advocate patient welfare, and serve as a liaison between patients, their families, and healthcare providers
Collaborate with new admissions, assess patient conditions and needs to develop personalized care plans
Evaluate plans of care and update them on an as-needed basis
Educate patients and their families on how to follow their care plans
Provide nursing care to their patients, if needed
Work with team to discuss cases and escalate concerns to the next level of responsibilities when indicated

Knowledge of each cancer diagnosis and its treatment related specifics
Communicate with physician and patient timely
Handoff communication with other team member

Lead or participate in quality improvement initiatives or projects
Promote the development of collaborative relationships with colleagues
Provide coverage and assist to clinic nurses with patient care activities when needed
- REQUIRED QUALIFICATIONS AND EXPERIENCES:
Registered Nurse with current Chinese practicing license
Minimal Bachelor degree in nursing
A minimum of 5 years of oncology nursing experiences; Outpatient oncology experience is preferred; Case management experience is a plus
Experience in using computer based health care information system is a must
Passion and drive to bring international practice standards to China
Fluent in spoken and written Chinese AND English
